What is MD+Chart?
MD+Chart is a medical community where together, we aim to improve electronic documentation and decision-making while working in a hospital or clinic setting. The website is dedicated to templates, key resources and encourages collaboration of great minds. It is a free resource dedicated to physicians in Canada and around the world. What is in the platform?
The Home Screen
Once you have signed into MD+Chart, you will automatically be redirected to the Home screen (shown below) of the platform. This is where we will be adding important updates, share some of the cool process maps that our members are coming up with and allow you to enter restricted parts of the website that is made just for you.
We tried to make the website as user friendly as possible. Medicine is a huge field, so we created 3 navigation systems within the platform.
- The search form is located in the right upper corner, beneath the billing and PPO shortcuts.
- MAIN NAVIGATION
- The main navigation menu is found in the left sidebar underneath the MD+Chart Logo.
- This menu will connect you with the different pages within the website that is hosting the templates, videos and a lot more.
- QUICK LINKS
- The Quick Links section is loaded with links to websites and tools that we use on a daily basis. These resources are located in all pages within the platform.
To reach the templates, simply select the department that you are working in. I will navigate the Hospitalist Section for this post.
Once you click on the specific department, the site will direct you to the templates we have created.
The following templates are available for hospitalists:
- History and physical
- Progress Note
- 3 x versions
- Discharge Summary
- Phone Call
- Critical Care
Our templates are designed to be imported into your voice recognition software or directly within your EMR. To copy the code, simply click on the Copy button and Paste it where you wish to use the template.
My personal preference is simply to leave the templates in the website and copy and paste them on the go. Another way to use the templates is to copy and paste it into your voice recognition software and then to create a macro, command or shortcut. We leave this up to your own discretion and what fits in with your flow.
Finding ICD-9 codes on Google can be time consuming although much more convenient than reading through a 100 page document and finding them manually. Our community is creating an easy way to find ICD-9 codes. Simply click on the ICD-9 Icon in the right upper corner. This will bring up the finder and you can start entering the word and retrieve the code you are looking for. If the code is not available, you can upload the code by clicking on the Add Code icon.
How To Create An Account
We welcome all physicians who wish to use our platform to open an account with MD+Chart. It is free and allows us to keep our community members safe. Click on the button below to get started.
Forgot My Password
We loose passwords all the time. If you have already have an account or our team created one for you, ask MD+Chart to send you a new one by clicking on the link below.
MD+Chart is not neurosurgery, but it can help you be more efficient and more goal directed when doing electronic notes. Trust me, it required months and months of trial and error to get to this point and if it can help us, then hopefully it can help you as well.

There are so many really helpful websites out there these days, so adding another one to your list may seem like overkill right now. I share your skepticism, so I would encourage you only to use our website if you wish to be more efficient in your day as a hospitalist or Emergency Physician. The Novel Coronavirus 2019, was first reported on in Wuhan, China in late December 2019. The outbreak was declared a public health emergency of international concern in January 2020 and on March 11th, 2020, the outbreak was declared a global pandemic. This new outbreak has been producing lots of hysteria and false truths being spread, however the data surrounding the biology, epidemiology, and clinical characteristics are growing daily, making this a moving target.
